Paper details:V. CAPSTONE PROJECT (3 Parts)As the major project for this course, the student will use the socio-ecological framework and prepare a complete program and evaluation plan that addresses a public health issue in a specific population. The plan will be grounded in the real world, and thus draw on real data and other information gathered in the needs assessment and planning processes.
Health Program Plan and Evaluation: Steps to Creating Your Capstone Project
Overview: This assignment is the Capstone Project for the concentration in Health Education and Health Promotion.
The purpose of the Capstone Project is to pull together what students have learned about conducting needs assessment, program planning, and applying social/behavioral theory to various levels (socio-ecological levels) and settings within the planning model. When applying these theories students are expected to design a culturally sensitive program, connected to a specific population and setting. The Capstone Project is worth a total of 400 points.
Procedure: The student will be provided with a Request for a Proposal (RFP) for a health promotion project. Students will develop a health education/promotion effort in response to the RFP, and present the program and its justification in a paper approximately 15-25 pages in length. The student must submit the CAPSTONE Project printed on one side of the paper and clearly tabbed in a three-ring binder, according to the component parts. The document is due on the last day of class.
Format: The Capstone project should be: 15-25 pages, double-spaced with 1” margins; The cover page and References page, both adhering to APA format, should NOT be counted in the 15-25 page limit.
1” margins
Times New Roman, Arial or Calibri font (no smaller than 12 pt.)
Double-spaced
APA Format, with correctly formatted in-text citations
Cover page
Sources used must be cited in-text
All sources must be listed on a separate page titled “References” at the end of the report; this must be in APA format
Please Note – failure to adhere to the APA format, grammar, punctuation, in-text citations, cover page, and references may trigger a drop in your final grade for the Capstone project. One of the clear competencies for a health education specialist is the ability to write effectively.
Plan Organization: The Part I assignment, Needs Assessment and Rationale, will cover sections A (Needs Assessment and Rationale) and B (Stakeholders). Part II. Program Plan and Implementation will cover sections C and D, and Part III. Evaluation will cover sections E, F, and G.
PART I. Needs Assessment and Program Rationale
Based on the material presented from the textbooks, the lectures, and tutorials, each student will independently conduct a needs assessment and prepare a rationale for their chosen health topic and target population/audience. Each student is required to submit a needs assessment, program rationale, and designated stakeholders.
4- 7 page summary of the needs assessment, program rationale, and rationale must be submitted at the beginning of the class period on the due date.
Below, students will find an example of an Outline for Part I of the Plan. Students may use this outline. Alternatively, you may use the Outline for an Evidence-Based Approach or the Social Marketing Approach from the Tutorials.
CAPSTONE PROJECT: PART 1 – Needs Assessment and Program Rationale
A. Instructions for Needs Assessment and Program Rationale
A.1. PREPARE TO WRITE YOUR RATIONALE – Review the four “Steps in Creating a Program Rationale” (McKenzie, pp. 21-35) in order to prepare to write your Rationale.
Step 1. From literature search, be able to explain past and current knowledge about your particular topic, including explanation of problem, approaches previously implemented by other planners and researchers in the reduction or elimination of the health problem, and people for whom the programs were intended (e.g. priority population);
Step 2. Think about the Title; It should be clear and concise.
Step 3. Outline the Content of the Rationale (see instructions below in A.2); and
Step 4. Prepare References in APA Format Used to Create Rationale.
A.2. WRITE the Program Rationale (McKenzie, p. 28) –(This is Step 3 above)
Writing Task 1. The first two paragraphs should identify your health problem from a global (e.g. international, national, regional, state) perspective. This portion comes from your literature review, epidemiological assessment, and synthesis of information.
(Hint: You are starting out broadly in your opening paragraph, continuing to narrow down to your specific population or audience of interest )
Writing Task 2. The next paragraphs should identify the problem at the level of your target (priority) population and show its relationship to the “bigger problem” at the levels you discussed in Step 1. This is referred to as your Problem Statement.
Writing Task 2.1. Identify a target population on which the needs assessment was conducted
Writing Task 2.2 Briefly discuss the scope and purpose of the needs assessment
Writing Task 2.3. Justify your choice of needs assessment activity/activities
Writing Task 2.4. State your method of data collection & analysis
Writing Task 2.5. Report results of needs assessment data analyses
Writing Task 2.5.1. Identify the priority health problem of your target population
Writing Task 2.5.2. Describe the factors (behavioral, cultural, social, and/or environmental) that contribute to this problem in your target population
Writing Task 2.5.3. Based on the literature review and your synthesis of findings, describe what determinants might be important in the development of this public health problem in your priority population
Writing Task 3. Propose a solution to the problem. The solution includes the NAME and PURPOSE of the proposed health promotion program, and a general overview or best guess of what your program may include (e.g., focus on teens who have not taken the HPV vaccine (e.g. purpose is to improve immunization rate of teens in the community, a “best guess” might be an intervention to increase awareness about the immunization, identify resources available to obtain the immunization, and reduction of barriers mentioned in the findings from the needs assessment)
Writing Task 4. Finish up the Needs Assessment and Rationale by writing a couple of paragraphs, discussing what can be gained from the program and why the program will be successful. For example, the program proposed is similar to several programs whose findings show that it was very effective. This is the place to use results of evidence-based practices to support your rationale. This is where you point out similarity of your population to other populations where similar programs were successful. This is where you point out that your stakeholders see this same need and/or why the timing is right to conduct this program to solve the priority population’s problem.
B. Identify the key decision maker(s) or stakeholders for your program (Simple List with Names, Agencies, Positions)
PART 2. Health Promotion Program and Evaluation Plan
Beginning with Part 2., students may elect to form a team and work together on the Program Plan and Evaluation. Rules: No more than three (3) students ma work together as a team. Students may not divide the work and assign different parts by student. Students must demonstrate that they are working together as a team. The team should have a timeline and plan with who is doing what and by when for each section. Building on the Needs Assessment and Program Rationale (Part 1, 4-7 pages), each student/student team will develop a Health Promotion Program and Evaluation Plan.
The professor strongly recommends that the Health Program Plan and Evaluation follow either of two approaches: the evidence-based program plan or the social marketing program plan, each of which you were assigned a step-by-step tutorial. Either of these tutorials will provide the student with a fail-safe step-by-step process. Part 2 should be 10-15 pages, minimum, and be composed of a detailed description of the Program Plan, and the Implementation. All work must be supported by scientific evidence and be based on at least one behavioral change theory, using the socio-ecological framework.
CAPSTONE PROJECT: PART 2 – Health Promotion Program and Implementation Plan
C. Describe in detail the Program Plan including:
1. Mission statement, Goals & SMART Objectives
2. Description and justification of the theoretical framework(s) selected for your program
3. Describe in detail what theoretical methods and practical strategies will be used to assist with the program planning and implementation; Support the use of your selected methods and strategies with a summary of evidence found in the literature
4. Prepare a matrix of program outcomes, objectives, methods & strategies and a brief description of what is included in the matrix
D. Describe in detail Program Implementation
1. Use the outline from the Approach you select (Evidence-based, Social Marketing, PRECEDE-
PROCEED, Systems Approach) to specifically describe how the phases of program
implementation will be/were applied
2. Include Resources, roles and responsibilities
3. Produce a Gantt Chart or Timeline
4. Prepare a Budget
PART 3. Health Promotion Program Evaluation Plan
Each student/team will develop a Health Promotion Evaluation Plan that is embedded in the Program Plan and Implementation. (Hint: Use your Logic Model to Guide the process) The Evaluation portion is built into the Logic Model and is based on the Program Plan. Recall that your SMART Objectives will be measureable.
CAPSTONE PROJECT: PART 3 – Health Promotion Program Evaluation Plan
E. Describe in detail, the Program Evaluation Plan for your selected public health problem (e.g., description of process)
1. Prepare your Outcome and Impact Evaluation
2. Discuss how the evaluation is directly related to the program goals and objectives
F. Results
[Optional and for Extra Credit]*
G. Conclusions (Required, regardless whether Part 3.F. carried out)
Based on your review of the evidence, explain why your plan is feasible and appropriate for your selected target population.
1. What are the limitations of your plan? What factors are not addressed in your plan (if any)?
2. Do you have any recommendations?
PLEASE NOTE*
Students are encouraged to implement the Program or a Unit of the Program and evaluate their Pretest or Pilot, reporting their findings.
